This learning module focuses on the clinical syndrome, frailty, in particular the concepts of loss of reserve, energy and well-being. Older people with frailty tend to present late, and often in crisis, so care can be hospital based, episodic and unplanned. This module argues that frailty should be reframed as a long-term condition, the trajectory of which can be managed proactively in primary and community settings by supported self-management and person-centred care.
