This learning module provides an overview of the barriers to providing patient-centred care and identifies how they can be overcome so that a patient's experience, values, needs and preferences in the planning, co-ordination and delivery of care are respected. Implementing a patient-centred care model improves outcomes, enables better use of resources, decreases costs and increases satisfaction with care.
