This learning module explores the use of structured tools for recognising a deteriorating patient. It also shows how nurses can improve their decision-making skills by using a Patient Assessment and Clinical-reasoning Tool (PACT) and ensure that care is patient centred by incorporating the 6Cs: care, compassion, courage, competence, communication and commitment. The ABCDE and SBAR approaches are also discussed.
